Developmentally Disabled (DD) Children's Rehabilitative Services (CRS) Program (DD-CRS)

Our Arizona DD-CRS Program offers a range of benefits for eligible members. Individuals with special health care needs may qualify based on medical condition and age. Members can see their specialists at one of our four clinics. They are located in Phoenix, Tucson, Flagstaff or Yuma. View coverage types below to see what benefits are offered.

Benefits & Features

Coverage Types

Individuals who qualify for DD-CRS have complicated health needs. We can put you in touch with someone who can answer your questions and and guide you. Call the number on this page to get in touch with one of our DD-CRS program specialists. The DD-CRS program is divided into two different coverage types

  • Partially Integrated-Behavioral Health. All CRS and behavioral health services are provided through UnitedHealthcare Community Plan. Primary care services are provided through the member's DD health plan.
  • CRS Only. All CRS services are provided through UnitedHealthcare Community Plan. Primary care services are provided through the member's DD or American Indian health plan. Behavioral health services are provided bythe member's RHBA or Tribal RBHA.

Get the care you need to be healthy — or to get better if injured or sick. Services will vary by the member’s coverage type and may include:

  • Medicines. Fill prescriptions at local pharmacies. 
  • Hospitalization. There are no copays for hospital care.
  • Transportation. We pay for rides to and from medical visits or the pharmacy.
  • Behavioral Health. We are concerned about how you feel with your mental health.

When you or your child have special and complex health care needs, you can depend on us. Our plan makes sure you get the specialized care, support and services that are needed. Services will vary by the member’s coverage type and may include:

  • Behavioral Health. Counseling, substance abuse counseling and other treatments are covered. Behavioral supports could include:
    • Behavioral health screenings, assessments and evaluations
    • Behavioral health prevention and health promotion
    • Living skills training and development
    • Supports to maintain employment
  • Care Coordination. Personal support from an experienced professional.
  • Asthma Support. Individual care to help control asthma or COPD.

Members recovering from a serious illness or surgery often need extra support.. Our plan includes the care and equipment needed to recover safely at home. Services will vary by the member’s coverage type and may include:

  • Personal Care. Services to aid healing after illness or injury. 
  • Equipment and Supplies. In-home medical and safety equipment are covered.

Sometimes members need a little extra help using their health plan. For those times, they can rely on:

  • Transportation. For DD-Partially Integrated BH members our plan provides trips to and from medical visits related to your CRS condition, trips to behavioral health visits and visits to the pharmacy.
  • For DD-CRS Only members our plan provides trips to and from medical visits and pharmacy visits related to your CRS condition.
  • Interpreters. Have an interpreter with you at your child's doctor visits.
  • Language Help. Translated materials are available at no cost.
  • Social Services. Sometimes a person's health and safety requires help from community programs. We help members connect to community services. Here they can get information about a variety of programs like:
    • Food and housing assistance
    • Community child care.
  • They can help members get much needed relief in many ways.
  • Member Services. Our member services team can help answer questions. They are available Monday - Friday from 8a.m. - 5p.m.

Care Management

Do you or a loved one have a serious health problem? If so, our care managers are in your corner. They can:

  • Explain medical terms in plain language.
  • Coordinate your doctor appointments.
  • Provide your care team with medical records. 

Your care manager can stay with you throughout the medical journey. He or she will:

  • Think beyond your medical needs.
  • Make sure you have support at home.

Services will vary by the member’s coverage type.

Choice of Doctor

All CRS members have the option of seeing their specialist at one of our four CRS clinics. They are located in Phoenix, Tucson, Flagstaff or Yuma.

Dental Care

Our plan will cover dental care related to the member's CRS condition.

Services will vary by the member’s coverage type.

Equipment and Supplies

Your health and safety at home are important.

Our plan covers medical equipment ordered by your doctor. This can include supplies like: 

  • Hospital beds.
  • Oxygen tanks.
  • Walkers or wheelchairs.

Services will vary by the member’s coverage type.

Hearing Care

Our plan will cover hearing care related to the member's CRS condition.

Services will vary by the member’s coverage type.

Hospitalization

Services will vary by the member’s coverage type.

This plan pays for expenses related to a hospital stay according to the member’s coverage type.

This includes:

  • Nursing care.
  • Supplies and equipment.
  • Treatment and therapies.
  • Diagnostic tests and exams.

Partially Integrated-Behavioral Health members receive
hospitalization services for their CRS condition and behavioral health needs through UnitedHealthcare Community Plan CRS.

CRS Only Members receive hospitalization services for their CRS condition through UnitedHealthcare Community Plan CRS.

Interpreters

Your child’s doctor and you need to understand each other. Not speaking English well makes this difficult. We can arrange for a medical interpreter to be at your child’s appointment.

We also have people at member services who speak more than one language. Chances are, we have someone who speaks your language.

Language Help

You can get information in your preferred language. Just ask. Then anything we write to you will only be in that language. This service is provided at no cost to you.  

Medicines

Our plan includes prescription drugs and refills. Coverage will vary by the member’s coverage type.

We also cover over-the-counter medicines with a written order from your doctor.

And we make getting your child’s medicine easy. You can fill your child’s prescriptions at:

  • Local network pharmacies.
  • Services that deliver right to your door.

Partially Integrated-Behavioral Health members receive pharmacy coverage for their CRS condition and behavioral health needs through UnitedHealthcare Community Plan CRS.

CRS Only Members receive pharmacy coverage for their CRS condition through UnitedHealthcare Community Plan CRS.

Member Services

Health insurance is complex. We can answer your questions simply and completely.

We help you find:

  • Doctors.
  • Pharmacies.
  • Hospitals or other health facilities.
  • Local resources.

Mental Health

Services will vary by the member’s coverage type.

Mental health is as important as physical health. 

Partially Integrated-Behavioral Health members will receive these benefits through UnitedHealthcare Community Plan CRS.  

  • Behavioral therapy.
  • Substance abuse treatment.
  • Medications.

CRS Only members can choose to receive these benefits through their Tribal RBHA.

NurseLine

NurseLine at 1-877-440-0255 available 24 hours per day / 7 days a week.

Respite Care

Partially Integrated Behavioral Health Members accessing behavioral health services can receive up to 600 hours per benefit year. (October 1st through September 30th)

Social Services

Sometimes a person’s health and safety requires help from community programs.

We help members connect to community services. Here they can get information about a variety of programs, like:  

  • Food and housing assistance.
  • Community child care.

They can help members get much needed relief in many ways. 

Transportation

Our plan provides trips to and from health care appointments.

CRS Only: Non-emergency transportation services through UnitedHealthcare Community Plan are available for a covered service related to the member's CRS condition. 

Vision Care

Our plan will cover vision care related to the member's CRS condition.
