The Florida Medicaid Plans offer a range of benefits along with extras that you may find useful.
Our Medicaid (MMA) plan covers children, families, pregnant women and single adults who may qualify based on income. UnitedHealthcare Health & Home Connection is a coordinated care management program for community-based elderly who are at risk of nursing home placement.

Alachua, Baker, Bradford, Citrus, Clay, Columbia, Dixie, Duval, Flagler, Gilchrist, Hamilton, Hardee, Hernando, Highlands, Hillsborough, Lafayette, Lake, Levy, Manatee, Marion, Miami-Dade, Monroe, Nassau, Polk, Putnam, St. Johns, Sumter, Suwannee, Union, and Volusia.
The MMA Plan is available in the following counties:
- Region 3: Alachua, Bradford, Citrus, Columbia, Dixie, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Putnam, Sumter, Suwannee, and Union Counties
- Region 4: Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia Counties
- Region 6: Hardee, Highlands, Hillsborough, Manatee and Polk Counties
- Region 11: Miami-Dade and Monroe Counties
The Health & Home Connection SMMC LTC Program is available in the following counties:
