You have options in getting the help you need. Find out how to apply and if you are eligible for Healthy Michigan Plan or another Michigan Medicaid Health Plan.

Once you qualify contact Michigan ENROLLS 1-888-367-6557 and ask to be a member of UnitedHealthcare Community Plan. If you do not choose a health plan you will be auto assigned to plan available in your county.
