Medicaid
Keeping our families healthy is a top priority for UnitedHealthcare. UnitedHealthcare works with the State of Virginia to provide high-quality health care coverage. Our Cardinal Care Medicaid plan covers eligible families, pregnant women, infants, children and adults. The plan also offers extra support and care to adults and children with a disability, long-term illness or special health care needs.
UnitedHealthcare Community Plan of Virginia offers a range of benefits along with extras that other plans don’t cover.

You are eligible for CCC Plus when you have full Medicaid benefits, and meet one of the following categories:
- You are age 65 and older,
- You are an adult or child with a disability,
- You reside in a nursing facility (NF),
- Individuals who receive services under the 1915(c) Commonwealth Coordinated Care. (CCC) Plus home- and community-based care (HCBS) Waiver
- Aged, blind or disabled (ABD) individuals, including disabled children and adults.
- Managed Care eligible populations who have Medicare (dual-eligible).
- Low-Income Families and Children Covered Populations, including
- Children under age 21
- Foster Care and Adoption Assistance Child under age 21
- Pregnant women including twelve months post delivery
- Infants born to a Medicaid-eligible individual
- FAMIS Children under age nineteen (19)
- Adults
- Under age twenty-six (26) who were formerly in foster care until their discharge from foster care at age eighteen (18) or older.
- Ages nineteen (19) to sixty-four (64) who are parents or caretaker adult relatives with a child under age nineteen (19) (MAGI adults)
- Ages nineteen (19) to sixty-four (64) who are childless (MAGI adults)

NurseLine
Medical questions and situations sometimes happen at inconvenient times. When you have questions about your health, you can call a nurse 24 hours a day, 7 days a week: 1-877-543-3409 (TTY 711).
Our nurses will:
- Listen to symptoms.
- Help with self-care.
- Advise you about getting care at a doctor visit or an urgent care center.
- Help you know when to go to the emergency room.
Transportation
Whether you live in the city or in the country, rides are available. Our plan provides up to 12 round trips to and from plan locations. This includes trips to and from the pharmacy to fill your prescriptions.

Translation Resources
We get member materials to you in a language or format that makes it easier for you to understand. Our provider network includes many doctors who speak different languages. Our provider directory shows which languages doctors speak. Additionally, many of our Member Service Navigators speak multiple languages. When you call, we will be able to connect with our interpreter service which offers support in more than 240 languages. We assist with interpreter services, including American Sign Language.
If you have trouble hearing over the phone you can use a text telephone. This free service allows persons with hearing or speech disabilities to place and receive telephone calls. Call 711. Give them the number 1-800-832-4643, and they will connect you to us.
You can also get information in large print, Braille, or audio files.

What to Do in an Emergency
Call 911 or go to the nearest hospital/emergency facility if you think you need emergency care. You can call 911 for help in getting to the hospital emergency room.
If you receive emergency services, call your doctor to schedule a follow up visit as soon as possible. Please call us and let us know of the emergency care you received.
An emergency is a condition in which you think you have a serious medical condition, or not getting medical care right away will be a threat to your life, limb or sight.
What to Do in a Behavioral Health/ Mental Health Emergency
You should call 911 if you are having a life-threatening behavioral health emergency. You can also go to a crisis center or the nearest emergency room.
You need to call Optum Behavioral Health at 1-888-872-4205 as soon as possible.
NurseLine
Medical questions can come up at time. When you have questions about your health, you can call a nurse 24 hours a day, 7 days a week: 1-866-801-4407, TTY 711.
Our nurses will:
- Listen to your symptoms
- Help with self-care
- Advise if you can get care at your next doctor visit or need an urgent care center

Healthy First Steps
Our Healthy First Steps program makes sure that both mom and baby get good medical attention.
We will help you:
Get good advice on nutrition, fitness and safety
Choose a doctor or nurse midwife
Schedule visits and exams
Arrange rides to doctor’s visits
Connect with community resources such as Women, Infants and Children (WIC) services
Get care after your baby is born
Choose a pediatrician (child’s doctor)
Get family planning information

Community Resources
Being healthy is not always about your urgent medical needs. Sometimes you may need help connecting to resources out in the community. We call these social needs. These are things like housing, getting food every day or feeling safe. Visit findhelp.org for a list of resources.
